What it does
Fabricate N lets you choose, when the creature enters the battlefield, to either put N +1/+1 counters on it, or create N 1/1 colorless Servo artifact creature tokens instead. It's an either/or decision—you can't split, taking some counters and some tokens.
The common mistake is forgetting it's a choice you make on entry, not a static bonus. If you do nothing, you don't automatically get the counters; you must pick. Also note the printed power/toughness already reflects the base creature—the counters or Servos are added on top. Servos are separate permanents, so they survive if the original creature dies.
In Commander, fabricate is flexible value: take counters when you want a bigger threat, or take Servos for wide boards, sacrifice fodder, or artifact synergies (Improvise, Affinity, modular). The token option also plays better against single-target removal.
